Abstract

The delivery of high-quality, reliable, affordable, patientcentered care has been a longstanding but elusive goal for most health care systems in developed and eveloping countries. Faced with mounting competitive, economic, and organizational pressures and challenges, health care systems have undertaken a myriad of efforts to transform existing institutions, practices, and policies. In fact, frustrated by the diffi culties inherent to improving key outcomes in health care, hospitals and other health care organizations have engaged in countless, often contradictory, attempts to change and to adapt.
